The following are items that we would like to review. Please check all items that apply.

 		 I have read this application and placement agreement, and have completed it in its entirety, and have
executed it with proper signature and financial information.
I understand the policies and procedures of the placement agreement you have chosen.

 		 I understand the nanny's weekly (salary or hourly) wage is negotiated between myself and the caregiver during
the interview, and their request wage is listed in their application.

 		 I have been advised and am aware that nanny applications sent to me are written in general terms; can
become outdated from the original information provided during the caregiver's interview; and the applications are
not written specifically for my family's job description.


 		 I understand that my placement may take longer than the normal time frame of (2) to (8) weeks, especially if
I am more "selective" in my caregiver's requirements: and I understand that all candidates registered meet
Childcare Resources' general requirements listed in the nanny's placement agreement.

 		 I understand the placement fees are non-refundable. I will receive a (6) month search and referral guarantee
according to the terms written in the placement agreement.

 		 I understand to submit an employment agreement to CRI according to the terms written in the employment
agreement, and firmly within (5) business days of the caregiver's date of hire.
  Note: We do not readily provide the caregiver's telephone numbers in their applications
(we are acting as their referral agent), but we will provide their contact information at the appropriate time.
